prior to your visit and/or surgery. Please download the appropriate form
as directed by your physician and/or staff by clicking on the name of

Appointments In order to better serve our patients, we can only see people
by appointment. Please arrive at least 15 minutes early so that we may register
you. If you need to cancel or reschedule an appointment, please call 24 hours
in advance.
X-Ray, MRI Films Patients with X-ray, MRI, or Bone Scan films must bring them
to their appointment, without these films you will be rescheduled. They are
very important in assisting your Doctor to determine the proper treatment for
you.
Second Opinion In addition to any films you have had taken, your Doctor
will also require any prior treatment records relating to the problem you are
being seen for.
Insurance Information We participate in most insurance plans, Medicare on assignment,
and most L&I and Self Insured Worker's Compensation Claims. Please bring
your health plan card to your appointments and provide us with all necessary
health care information. If your insurance plan requires an authorization from
your primary care provider, it is your responsibility to obtain this prior to
your appointment.
Billing Copayments are due at the time of service. For your convenience,
we accept cash, checks, Visa, Mastercard and American Express. We electronically scan our checks.
Prescriptions We require at least 24 hours notice for any prescription
refills. Please call your pharmacy and ask them to fax us a refill request to

Insurance / Disability Forms Please allow 7 to 10 working days to process any insurance
and/or disability forms that you may have filled out. There will be $10.00
charge for forms not covered by insurance.
